Leslie's 40th Birthday Dance-Fit Benefit for God's Misfitz

Join Leslie for an epic  40th birthday celebration! Wear your black and neon for this fun and energetic dance workout! Our class is featuring Dontrell Johnson, CEO of Greatness Elite Training. All proceeds will be donated to God’s Misfitz, a local nonprofit organization dedicated to serving our unhoused brothers and sisters living on the streets of Albuquerque, NM. All are welcome to join in on the fun! 

Dance Fitness Waiver: 

By registering for this event, I hereby acknowledge and understand that participation in fitness classes and related activities conducted by Leslie D. Ricks, Dontrell Johnson, God's Misfitz, and Impact New Meixoc Studio involves certain risks and potential dangers. By registering for this event, I voluntarily agree to participate in the class and assume all risks associated with my participation.

Medical Representation: I represent that I am physically fit to participate in the fitness class and have no medical condition(s) that would prevent my safe participation. If I have any medical conditions or concerns, I have consulted with a healthcare provider and obtained clearance to participate.

Consent to Medical Treatment: I hereby consent to receive any necessary medical treatment resulting from my participation in the fitness class and agree to bear all costs associated with such treatment.

Photography and Video Release: I hereby grant permission for Leslie D. Ricks and OIG, LLC to take and use photographs and videos of me for promotional purposes without compensation.
